the Board as well as parents, guardians and service providers. These policies
and procedures are available in any Wood Lane Office. If you have any questions,

Services Waiting List:
A request can be made by an individual or his/her
guardian to be placed on the waiting list. An assessment of needs will
be completed by the individual, Individual Planning Team, Service Coordinator,
or parent/guardian.

DNR (Do Not Resuscitate):
Each year Wood Lane reviews its DNR (Do Not Resuscitate) policy. The policy allows
families/guardians to request a DNR in specific situations. This policy
is not taken lightly. Due to the sensitive nature of this policy, the
Board requires an annual notification to be given to all staff, individuals
served, parents of minors, and legal guardians about the existence of this
policy.
